Insurance cardPayor, member ID, plan, group number. Front/back photos travel only with Smart Health Link, not with Smart Health Card.
Recent appointmentsLast few visits with date and provider.
Recent vitalsBlood pressure, weight, glucose, etc.

The QR code contains the data above signed by TendTo, but it is not encrypted. Anyone who scans it can read the contents. Only show it to the clinic at check-in.
