Pledge — Objective
Provide tailored, data-driven support to individuals at risk for or living with diabetes and obesity, powered by direct access to clinical data from trusted networks.
Criteria coverage
- Connect to a CMS Aligned Network. Direct connection to Medicare Blue Button 2.0 via OAuth 2.0 with PKCE. Imported FHIR R4 resources (Observation, Condition, MedicationStatement, ExplanationOfBenefit) are the basis for the trends and coaching surfaces below.
- Use the clinical record to generate personalized coaching, reminders, and risk alerts.Tali (TendTo's AI care companion) reads HbA1c, fasting glucose, BMI, blood pressure, lipid panel, and weight trend lines from the imported Observation resources and surfaces personalized guidance in chat — paired with the existing CMS CAI disclaimers.
- Adapt support for both prevention and active management.Tali detects the patient's current state (no diagnosis, prediabetic A1c 5.7-6.4%, or active T2D) from the Observation/Condition data and tailors coaching language, target ranges, and alert thresholds accordingly.
- Specifically provide appropriate resources for patients who are pre-diabetic. The Health Trends page surfaces a CDC National Diabetes Prevention Program (NDPP) referral panel when the most recent A1c is in the pre-diabetic range. Caregivers can save the referral as an appointment task and Tali can answer questions about the program.
- HIPAA-aligned operation. All data flows through the same RLS- scoped Postgres tables and BAA subprocessors as the rest of TendTo. No third- party coaching vendor receives PHI.
Surfaces
- Health Trends (/health-trends): trend charts for A1c, fasting glucose, BMI, weight, blood pressure, and cholesterol. Pre-diabetic risk panel and NDPP referral when applicable.
- Tali coaching (/dashboard): chat- based guidance on diet, activity, medication adherence, and when to escalate. Disclaimers per CMS CAI pledge.
- Pre-Visit Packet (/pre-visit): imported labs and conditions ride along inside SMART Health Cards / Links so the patient can carry their own evidence to the next visit.
